The Discrimination, Harassment, Sexual Misconduct, and Retaliation Reporting Form is used to notify the Civil Rights and Title IX Office of any incident or conduct that may be a violation of the College’s Equal Opportunity, Non-Discrimination, & Discriminatory Harassment Policies or Sexual Misconduct Policy. This includes, but is not limited to, incidents of discrimination, harassment, sexual assault, domestic or dating violence, stalking, or other forms of sexual misconduct.
